Couple things:
1. Looks like he has the front doors on, so therefore should be within CG.
2. Cannot tell if front left seat is occupied.
3. One guy in back.
4. Two on the line. So assuming 180 lbs each....plus two inside, should be within weight limits
Looks like one of two things, 1. Pilot has never done external load operations before and picked them up swinging. 2. Started a deliberate swing, and did not take into account inertial forces and ran out of pedal and got LTE which would account for the spinning helicopter.
