Very interesting.
15 holes in the cheese lined up, almost any one of which not doing so could have prevented the accident. OTOH, an operational sortie like this probably has 5 or 6 different 'holes' lined up every time. Fatigue typically affects fine visual judgement, but in this case the physical light levels were not there for the pilots to be able to make sufficiently fine judgement anyway. Probably the best move, given the lack of recent currency, would have been to let the experienced guy fly as AC. That's my 5c.

Looks more like RATOG, wonder why they did it as the take off was not particularly short
