Further, in American culture it is a sign that means 'okay', and I've never associated it with, or heard of it being associated with white supremacy here. For example, a proud father might flash it at his son who just won an award or athletic achievement. Or it could be used when you just took a tumble and wanted to signal to others that you are all right and not hurt.

That could very well be true. As I have never met a white supremacist I wouldn't know about that. What I wrote about that very natural gesture is also true, and that it how it is most widely used in the American culture, and I'm confident that usage was what President Trump had in mind.
