Wow, is all I can say….look at ALL the holes in the Swiss Cheese……… I will have a whole bunch to say on this later today, I’m about to head out the door……

So in reading the article there were so many red flags and holes in the cheese:
  • Pilot just off of days off
    In experienced pilot—-not sure his experience level
    Two circuits from different companies
    Failing weather
    No wire cutters on the aircraft
    Black and white maps
    One circuit not depicted on maps
    Wires were un-marked
    High workload
    They knew the wires were there
In my job, we now patrol 250’ above the circuits and line men can still spot problems. Then they can pull out gyro stabilized binoculars with built in cameras to further investigate. We would typically then consider landing and having the line men inspect from the ground, or, bring in a drone team.

Recently watched line inspectors nearby who also seemed to be flying at at least 250', probably more. I went onto the front verandah with camera and long lens when they'd finished. As I looked through the viewfinder, I could see one of the observers looking back at me with an even longer lens.
